We understand that over the course of their learning an Apprentice can change their name for a variety of reasons however we have a duty to ensure that we issue certificates accurately. Therefore we may require you to load additional evidence to MAO that confirms the name change. Acceptable forms of Name Change Evidence include:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li>Marriage Certificate<\/li>\n<li>Civil Partnership Certificate<\/li>\n<li>Gender Recognition Certificate<\/li>\n<li>Enrolled Deed Poll<\/li>\n<li>Act of Parliament<\/li>\n<li>Unenrolled deed poll\/Change of name deed<\/li>\n<li>Certificate from the Court of the Lord Lyon of Scotland<\/li>\n<li>Birth Certificate (upon re-registration)<\/li>\n<li>Baptismal\/Confirmation certificate (for Christian names only)<\/li>\n<li>Certificate of Naturalisation\/Registration<\/li>\n<li>Adoption order\/certificate (these will not normally be provided as a change of name evidence)<\/li>\n<li>Statutory Declaration\/Affidavit<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p style=\"text-align: justify;\">It is very difficult to produce comprehensive guidance on the range of evidence above. We have also agreed that we would accept a signed statement from the Apprentice which are also countersigned and dated by someone in authority. This person in authority should not be an employee of the apprentice\u2019s appointed training provider and the most common counter signatory used successfully is the Apprentice\u2019s employer contact. These statements can also be used to confirm discrepancies between <b>Dates of Birth<\/b> and <b>Gender<\/b> on achievement evidence and the details held in MAO. The supporting evidence should be signed and dated by the apprentice <strong>and<\/strong>\u00a0the employer.<\/p>\n<h1>National Insurance (NI) Numbers<\/h1>\n<p>In October 2017, <a href=\"https:\/\/modernapprenticeships.org\/web\/ni-number-validation\">a new validation for NI numbers was introduced to prevent the increasing number of duplicate NI numbers appearing on the system.<\/a> If an Apprentice has been entered onto MAO previously, the forename, surname and NI Number fields for any new records will need to match the existing record exactly. If you enter an Apprentice and the NI Number matches another record in a different name you will see an error message that will prevent you from adding the details to MAO.<\/p>\n<p>&nbsp;<\/p>\n<p>If we find any major discrepancies between NI Numbers we may ask you to provide evidence of your apprentice\u2019s NI Number or change of name documentation before you can register them on MAO. <b>Statements signed by an Apprentice cannot be used as evidence of the correct NI Number<\/b>. Examples of acceptable evidence for NI Numbers is as follows:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li class=\"first-child\">P45 or P60<\/li>\n<li>Letter from HM Revenue and customs confirming your National Insurance Number or regarding tax or tax credit.<\/li>\n<li>Bank statement showing payments by direct debit for class-2 National Insurance contributions, or benefits payment received showing your NINo on the statement, for example, Incapacity Benefit or State Pension.<\/li>\n<li>Payslips, salary statement or works pension statement (as long as it shows your NINo)<\/li>\n<li>Letter that the Pension Service or Jobcentre Plus has sent to you (not handwritten)<\/li>\n<li class=\"last-child\">NINo Card (not handwritten)<\/li>\n<\/ul>\n<p>Created 03.05.17<br \/>\nUpdated 07.12.17<\/p>\n","protected":false},"excerpt":{"rendered":"<p>MAO is designed with the idea of apprentice\u2019s progressing through a number of different apprenticeships throughout their career.
